By Submit Registrstion From, you agree that:

1. Bind by and follow our league rules.
2. Agree on Player’s Waiver

The undersigned player or parent / guardian of the above named player, hereby release the torontochinese.ca Sports club, the club staffs and instructor from blame or fault for any accident arising from said player’s participation in the club’s activities, including games, practices, or while en route to or from such games, practices, or other activities. A team security deposit $200 required at the beginning of season, and will be returned at end of the season if the team did not have a record of damaging the property nor any foul behavior (fighting etc.) Furthermore, In consideration of the player’s participation in such programs, activities and events, I hereby acknowledge that I am aware of the risks and hazards associated with or related to basketball. Any injuries, damages, losses, costs, liabilities or expenses will be at the undersigned player or parent’s liability.

I acknowledge that I have fully read this agreement, that I have executed this agreement voluntarily, and that this agreement is to be binding. I agree to abide by the rules and regulations of the torontochinese.ca Sports club.
